Billing & Insurance
Please be sure to provide us with complete and accurate information about your insurance, and we will submit claims to your insurance carrier and receive payments for services. We recommend you contact your insurance carrier to confirm your benefits and understand your out-of-pocket responsibilities. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts.
To pay these out-of-pocket amounts, we recommend you log into your Patient Portal at https://colonycare.sigmundemr.com. Please use your email and password created upon registration to access the portal. In the portal, click the Billing icon to view your statement and make payments securely.
To make payment by phone or ask any questions regarding your statement, please contact our dedicated billing service line at (630) 789-9103.
