Billing & Insurance

Please be sure to provide us with complete and accurate information about your insurance, and we will submit claims to your insurance carrier and receive payments for services. We recommend you contact your insurance carrier to confirm your benefits and understand your out-of-pocket responsibilities.  Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts.

For any dates of service prior to January 1, 2024, please pay out-of-pocket amounts through the Aura Elevate Patient Portal at https://colonycare.sigmundemr.com.  Please use your email and password created upon registration to access the portal.  In the portal, click the Billing icon to view your statement and make payments securely. With any questions regarding your statement for dates of service prior to January 1, 2024, please contact our dedicated billing service at (630) 789-9103.

For any dates of service on January 2, 2024 and after, please pay out-of-pocket amounts through the AdvancedMD Patient Portal at https://pp-wfe-102.advancedmd.com/156401/account/logon.  Please use your email and password created upon registration to access the portal.  In the portal, click the Bills icon to view your statement and make payments securely. We ask all clients to keep a form of payment on file by marking the checkbox to do so on the Pay My Bill tab. With any questions regarding your statement for dates of service on or after January 2, 2024, please contact our dedicated billing service by sending a message through the patient portal with the subject “Billing & Payments.”
